This form is somewhat exhaustive and will require 15 – 30 minutes to complete.
Client will need to bring the following documents to 1st meeting with SSVF Staff:
- Copy of DD 214
- Proof of Current Income
- Copies of any overdue bills client needs assistance paying
- If in housing, copy of an eviction notice and lease agreement
- If HUD-VASH, copy of letter stating they are a VASH client
- ID Card, Social Security Card and Marriage License, (if married)
